Intervention Presentation and Capstone Video Reflection

Hi, my name is ______. This presentation concerns the intervention I have planned and got approved for to be implemented for patients with heart disease. This intervention seeks to assist them through a holistic approach covering their medical, psychological, financial, and social issues. Throughout this practicum, I have dedicated ample time to focusing on the patients and gaining advice from professionals to guarantee that the intervention will be helpful. I have gained a lot from this experience because there is no better way of realizing the effect of your work on patient care outcomes than taking a close look at it. While studying in the RN-to-BSN program, I have experienced a lot of professional and personal growth. In the video reflection, I have discussed my practicum placement, my capstone project development, and my growth throughout this course.

Contribution of Intervention on Family Satisfaction and Quality of Life

The process of care for patients with heart disease was to improve the quality of the delivered care and the patient experience. The healthcare, emotional, and financial needs of patients with heart disease were met through the interventions. Engaging patients in every aspect of the care plan helped in understanding their concerns and it enhanced their trust and satisfaction. In this case, the application of evidence-based practices and innovative technologies, such as ICDs and Apple Watch, was vital for offering adequate treatment modalities (Ojha, 2022). Furthermore, the interventions provided psychological care to enable patients to manage the stress related to heart disease and also acquire healthy habits that promote heart and overall patient health. 

Quality of Life

Through the heart disease patients’ constant feedback and self-evaluation, it is evident that the perceived quality of life was significantly enhanced through the proposed interventional plan. The care plan customized to patients’ unique needs covered their current health concerns and also their future health conditions. Self-care resources such as nutrition and psychological consulting services and educational resources on the management of heart disease made patients active in making the right choices for their lifestyles (Williams et al., 2020). Implementations were regularly followed up with professional healthcare givers to ensure that the intervention met the intended goals of patients. The feedback received from the patients emphasized that they valued the treatment process because it was person-centered which made them feel cared for throughout the process.

Patient’s Family Satisfaction

According to the feedback from the patients with heart disease, they showed satisfaction with the treatment interventions for their heart problems. They valued the one-on-one approach by healthcare providers. The implemented care plan includes healthcare treatment, counseling, and financial support for patients (Siconolfi et al., 2021). The interventions reduced patient satisfaction and communication gaps. The interdisciplinary healthcare team played a crucial role in achieving these goals. 

Evidence for Planning and Implementing Capstone Project

The capstone project utilized evidence-based studies to build improved treatment interventions for patients with heart disease. Recent research by Rossi et al. (2023) on heart disease management showed the importance of community support and psychological counseling in improving patients’ healthcare. Furthermore, research by Wray et al. (2021) shows that when healthcare providers collaborate with community workers in the treatment plan, it increases patients’ satisfaction and awareness about their health conditions. Furthermore, the KardiaMobile EKG – an advanced technology device also helps in monitoring the patient’s heart data and reduces the chances of critical situations. These evidence-based interventions can help healthcare providers decrease hospitalization rates as complications are treated before they even rise.

Incorporating Professional Guidelines

Professional guidelines were a crucial part of developing interventions for patients with heart disease in this capstone project. Advanced Practice Nurses (APNs) serve as a guide to improve the treatment of patients with heart disease (Advanced Practice Nurses, 2021). Based on clinical practice and standards, these guidelines help healthcare providers to ensure patient care and satisfaction. Furthermore, these guidelines ensured that patients’ rights including autonomy, non-maleficence, and beneficence are also protected. 

Evidence-Based Practical Interventions

The developed interventions are rooted in updated evidence-based practices that prioritize patient’s healthcare needs. Research by Belardo et al. (2020) stated that patients’ health improved when evidence-based interventions were a part of their treatment plans. Furthermore, using active listening and patient feedback allowed healthcare providers to be there for the patients and cater to their needs. These evidence-based interventions proved to be effective for patients with heart disease.

Principles of Evidence-based Practice

The evidence-based practice allows healthcare providers to develop credible nursing interventions for the treatment (Smith et al., 2022). That is why this capstone project focused on developing improved, patient-centered, and relevant interventions for patients with heart disease. The ethical standards were assured throughout the project. Patient’s healthcare experience was also improved by involving them in treatment discussions and making decisions based on their preferences. 

Healthcare Technology to Improve Health Outcomes or Communication

As the world is advancing every day in technology, such advancements in healthcare are also used in this capstone project to provide improved quality treatment to patients with heart disease. The EHRs were used in the treatment plan to keep patient data and information protected and easily accessible (Vos et al., 2020). To provide efficient care for patients, healthcare providers focus on better communication and collaboration within interdisciplinary teams. Incorporating telehealth devices such as the Apple Watch that provide accurate data on patients can decrease hospitalization rates. These technologies also assisted in improving communication and treatment interventions for heart patients at affordable costs. 

Furthermore, using mobile health applications allows patients with heart disease to have better strategies for managing their health. Self-monitoring applications for heart rate and lifestyle modifications provide timely information (Arkee et al., 2021). This was beneficial for the patients as they were given more control over their treatment and for healthcare providers as they could follow up with their patients and take action when needed. 

Improvement in Health Care Technology Use

Although the current implementation of healthcare technology within the capstone project showed positive outcomes, there is always room for advancement. A potential area of improvement is in the application of more sophisticated data analysis methods (Krittanawong et al., 2021). Devices like the CardioMEMS HF System have greatly lowered readmissions for heart failure cases because these devices constantly monitor pulmonary artery pressure to signal any deterioration before symptoms are observable (Vyas et al., 2023). They could be used to analyze the patient’s data and offer accurate prognoses and treatment plans depending on the health status of every patient. This allows patients to customize care approaches that address their specific medical history and other aspects of their lives. 

The other possible improvement area can be the incorporation of artificial intelligence (AI) solutions to address patients’ needs. elf-service technologies such as AI-anchored chatbots and virtual health assistants could be beneficial for patients because they would give constant availability of information and guidance on handling cardiac stressors (Yousuf et al., 2024). These technologies can also help in appointment making, giving reminders on medication intake, and sharing educational materials to ensure patients’ improved healthcare.

Opportunities for Future Improvement

Identifying potential areas for increasing the use of healthcare technology in practice is crucial for the patient’s care. An important one is to introduce more advanced artificial intelligence and machine learning technologies to offer predictive analysis and patient-tailored recommendations (Chowdhury, 2024). or heart disease patients in particular this could lead to more personalized and comprehensive patient care that changes in response to the patient’s needs and progress. Moreover, the use and application of wearable technology can be extended to offer continuous patient monitoring which will be informative to both the patient and the health professional.

Other areas of improvement that were identified include improving patient information and involvement. The use of awareness workshops makes it possible for the patients to be exposed to highly engaging conceptual training on their diseases and their treatment (Harvie, 2021). For patients, engaging in awareness workshops by community services can reduce anxiety and improve their engagement in the decision-making process. 

Influence of Health Policies in Planning and Implementation of Capstone Project

The implementation of healthcare policies helped influence the formation and management of the capstone project concerning patients with heart disease and its risk factors. ACA makes the interventions more accessible and affordable by providing insurance to cover specific heart disease treatment costs (Baumgartner et al., 2020). Furthermore, developing interventions based on ethical grounds was possible due to the guidelines of Advanced Practice Nurses (2021). Such reforms and guidelines help healthcare providers to address the healthcare and emotional needs of patients. 

Contributions of the Capstone Project to Policy Development

This project is beneficial in developing new policies because it found certain gaps that limit the healthcare for patients with heart disease. The patient’s data and feedback provided valuable insights into identifying the support that policies provide to patients with heart disease. These pieces of evidence supported the call for increased insurance coverage for patients with cardiovascular disease and the required coordinated mental health services (Daumit et al., 2020). The project’s results highlighted the feasibility of receiving community support and integration with an interdisciplinary team to discuss policies and improve the quality and accessibility of care for patients diagnosed with heart disease.

Role of Nurses in Policy Implementation and Development

Nurses with a bachelor’s degree must play a crucial role in effectively advancing and fostering the implementation of health policies. In this capstone project, patients with heart disease were provided support by nurses. These policies enable the nurse to ensure the patient gets the right care that corresponds to the policies in health and fills the communication gaps between the patients and healthcare providers. Moreover, the nurses helped in policymaking by offering essential information on practices on the ground and patient outcomes, which was used to modify policies and make policy recommendations (Locock et al., 2020). This role where the nurse is both the implementer of healthcare policies and an advocate demonstrates the essential function of nurses in the change process of policies to serve the patients and improve the healthcare system.

Capstone Project Outcomes Compared with Initial Predictions

The results of the capstone were aligned with the expectations outlined at the beginning of the process with some aspects outperforming them. The first purpose was to address heart disease and its stressors in patients and to offer them well-founded support in this matter. The expected results were positive changes in the psychological well-being of patients, their awareness of available treatment options, and their satisfaction with the healthcare interventions. These goals were achieved and the patient’s feedback provided evidence for it because their wellbeing was improved. These outcomes support the project’s effectiveness in improving healthcare interventions for patients with heart diseases.

Generalizability of the Intervention

The intervention designed for cardiovascular disease has the potential to be effectively implemented in other settings. Such a structured plan for offering emotional and informational support can be applied to patients with other diseases like diabetes (Bardhan et al., 2020). Counseling sessions, educational materials, as well as dietary modifications can be implemented across interventions. Furthermore, the outcomes found in patients with heart disease indicate that similar measures should be carried out in other health facilities, as it will improve the quality of care offered to patients. 

Best-Practice Potential of the Intervention

The method used in the capstone initiative proves that it can be useful as a best practice for handling health problems mainly those that are related to heart diseases. The counseling and awareness interventions proved beneficial in improving the patient’s coping mechanisms and well-being. These interventions align with the best practice principles because patient-centered care is a top priority, patient safety is improved by the knowledge gained through involvement in the decision-making process, and the total cost of care is also minimized (Mapes et al., 2020). This means that the intervention was effective in these cases of patients with heart diseases and that it should be used effectively in practice for better results and improved quality of patient care. It also serves as a model that can be implemented in other clinical settings to enhance the overall patient experience.

Conclusion

The capstone project demonstrated an integrated approach to improve emotional and informational support for patients with heart disease. The outcomes of the project were subsequently in touch with the expected results indicating efficacy in enhancing better emotional state and favourable health care results. In addition, the results of the intervention apply to other patients with similar problems, suggesting that it has the potential to become a model intervention in the field of cardiology. Furthermore, such evidence-based interventions must continue to be explored in the future to help advance patient-centered care and advance the prognosis for cardiovascular disease clients.
