Waiver and Consent Form

Institution: Capella University Course: NURS4015 or NURS-FPX4015

Lisa Smith (“party”) freely agrees to take part as a mock case in a health assessment video demonstration conducted by Brianna (“pupil”), a nursing learner at Capella University. By subscribing to this form, I admit that I understand and accept the terms below.

Purpose of Participation

I understand that the recorded content will be used simply for educational objects. These include

• Demonstrating health assessment ways and clinical chops for academic evaluation.
• Completing a comprehensive examination supported by a private, ideal assessment and plan (cleanser) note, as demanded in the course class.
• furnishing academic health data for a simulated clinical assignment.

I admit that I waive the right to review or authorize the material prior to its use by Capella University.

Content Agreement

I assent to being video-recorded for the development of course-related paraphernalia. The “Content” includes my image, voice, likeness, words, and appearance, as captured in the recording, as well as any information I give to help the pupil in completing the cleaner note.

Disclosures

I understand that.

• The information captured is solely for demonstration and doesn’t constitute professional medical advice or opinion.
• Neither the pupil nor the party is required to partake in factual medical history or private health details.
• Except for general demographic data (analogous to age and gender), particular identifiers may be fictitious for the purpose of the assignment.
• Certain vital signs or simulated health readings may reflect my factual health information.

Voluntary Consent and Use

I freely grant Capella University full, royalty-free, and irrevocable rights to use the Content for educational purposes. This includes, but is not limited to, sharing with the course preceptor, applicable faculty, or staff.

I further waive the right to

• Check or authorize the content prior to its use.
• Make claims for damages or compensation related to the use of the Content, including differences or variations of my likeness, voice, or appearance.

Rights and Ownership

I admit that Capella University will retain full and exclusive power of all content created under this agreement. The material will be considered the property of the university.

I hereby release Capella University from

• Any claims related to the power, creation, or use of the Content.
• Legal claims involving hype rights, insulation rights, defamation, or damages.
• Any damage, loss, or figure may be due to the product or education of accoutrements.

Waiver and Release

I fully release and don’t agree to apply legal conditions against Capella University, its cells, workers, contractors, scholars, or products, or the use of accoutrements.

Governing Law and Venue

This impunity will be decided by the laws of the State of Minnesota, and any disagreement will only be resolved in the Minnesota State or Civil Courts.

Confirmation of Consent

By subscribing below, I confirm that I’m at least 18 years old, that I’ve read and understood this impunity duly, and that I freely agree with the conditions mentioned therein singly.
